| 1. All Arrow STP Lab's tools and equipment MUST be used inside Arrow STP Lab. |
| 2. The borrower is responsible and liable for the equipment that is borrowed from Arrow STP Open Lab. This liability includes replacement and repair costs associated with the equipment loss or damage. Borrower loans must be authorised by the appropriate Arrow staff and in some cases borrower may need to undergo a proficiency test before a loan will be authorised. All equipment loans are subject to the Conditions of Loan and Use policy of Equipment Loans Policy at Arrow Open Lab. |

I / We, the undersigned, hereby apply for membership of Arrow Open Lab and agree to abide by the rules and regulations of Arrow Open Lab. I / We declare to the best of my / our knowledge that the information given in this application form is true and correct and all the supporting documents are true copies of the originals.
